Birth Registration & Turkish ID Card Application (Application can be made either in person or via mail）

To apply, please submit the documents explained below (1-7)

Either mother or father must be Turkish citizen.

1)PetitionDOWNLOAD FROM THE TURKISH PAGE

* All sections must be filled and signed by Turkish parent.

* In your petition, please explicitly mention if your child holds the citizenship of foreign country, and write his/her official name and surname as in the passport. In addition, if the child has foreign passport attach the copies to the petition.

2)Passport copies of the parents(Passports should be valid. If foreign parent is not a passport holder, valid driver license or health insurance card are also acceptable.)

3)Sealed copy of SHUSSEI TODOKE

4)SHUSSEI JURI SHOMEISHO(original)

5)If one of the parents has Japanese citizenship,KOSEKI TOHON (original one with child’s birth record)

6) Application fee 1251 -JPY
(for Turkish ID card)

7)ThreeRed Letter Packs(Pre-paid return envelope) (Please write your postal address to receiver section)

Important note:

* If the parents are not married,application should be made in person. Both parents are required to apply personally and accompanied by one of oursworn translatorsfor child recognition transactions at the Embassy or Consulate General.

* If the mother is foreigner, she must submit an official document proving that she was not married or she was divorced 300 days before the birth.
